Please bring a photo identification card, a copy of your dental insurance card, and referral slip with any x-rays your dentist may have given you.  If under 18 a legal guardian must accompany patient to initial appointment. If you choose to schedule an appointment to start treatment and then decide to have only a consultation, there will be an additional fee for the block of time that has been reserved for you. Fee is 75.00 in addition to the consultation fee.

Please alert our office if you require a pre-med prior to dental treatment even if procedure is not scheduled that day.
